Welcome to community care and rehabilitation

Community Care and Rehabilitation has a number of outpatient ambulatory care programs that provide multidisciplinary team-based care including:

  • Bariatric Regional Assessment and Treatment Centre 
  • Cardiac Rehabilitation 
  • Pulmonary Rehabilitation, 
  • Chronic Disease Self-Management Program 
  • Diabetes Education and Care
  • Driver Assessment and Rehabilitation Service (DARS) - service temporarily unavailable
  • Hemophilia Care 
  • HIV / AIDS / Hepatitis Care (HAVEN Program) 
  • Integrated Chronic Pain Program 
  • Outpatient Brain Injury Rehabilitation  
  • Outpatient Physiotherapy 
  • Hand and Upper Limb Therapy 
  • Seating and Mobility Clinic
  • Prosthetics & Orthotics 
  • Assistive Communication Clinic 
  • HSN Speciality Clinics
  • Northeastern Ontario Regional Stroke Network

The program is also responsible for the Inpatient Rehabilitation Unit – 3rd floor South Tower at Ramsey Lake Health Centre. 

 

The Diabetes care service provides self-management education and support to people living with diabetes. The multi disciplinary team of nurses, dietitians and social worker provides instruction on nutrition, exercise, insulin management, blood glucose monitoring and many other health topics.

 

The Hemophilia program provides support and education to individuals living with inherited bleeding disorders, such as hemophilia and Von Willebrand’s disease.

The team of health professionals includes a nurse coordinator, hematologist, social worker and physiotherapist, who work together to help patients manage their condition and provide them with the best available treatment options.
