We serve people who were born with a disability or got it later in life and now have trouble moving around. Our team of experts will work with you to figure out the best way to help you move and sit comfortably while being mindful of your complex needs.

If you have a valid OHIP card, the cost of the assessment is covered.

The cost of the equipment is not covered; however, you may be able to receive funding through the Ministry of Health’s Assistive Devices Program. Visit their website for more information.

We do not sell wheelchairs, seating, or positioning devices. Our clinic can work with your preferred seller to be sure the items fit your needs.

If you’re unsure where to buy your equipment, we can provide a list of vendors on request.

Only physicians or nurse practitioners can submit a referral to the Seating and Mobility Clinic.
 

 Provider Referrals

Healthcare providers are asked to complete the following documents and fax them to the Seating and Mobility Clinic at 705-523-7051.

• Seating and Mobility Clinic Referral Form

If you meet the eligibility for the clinic, you will be placed on a wait list and contacted when the next appointment is available.
