Heart Failure Disease Management Program

Heart failure is defined as the inability of the heart to pump adequately to meet the needs of the body. The team in the Heart Failure Disease Management Program provides comprehensive care for heart failure patients throughout the course of their treatment. The program is one of very few in Northeastern Ontario. We also have a new Cardiac Amyloidosis Clinic that screens and treats patients for this rare disease that leads to heart failure. We offer outpatient consultation and follow-up for patients either in-person at Health Sciences North or via videoconference (Ontario Telemedicine Network) if the patient’s condition warrants.

The program’s core team provides complex management of heart failure and includes cardiologists, one nurse practitioner and a highly skilled team of registered nurses experienced in heart failure. Together, the team is responsible for all aspects of your care.

The inter-professional team approach is needed to:

  • Optimally diagnose the patients’ condition.
  • Carefully review and prescribe treatment.
  • Educate and counsel patients and families regarding medication use and lifestyle changes.
  • Provide advance care planning.
  • Provide post-discharge follow-up for patients recently discharged from the hospital.
  • Accommodate requests for urgent consultations.

A referral from a physician or Nurse Practitioner is required.  Patients in the program are seen by a cardiologist OR a nurse practitioner and may rotate between the two from appointment to appointment. You may see a nurse practitioner for two to three appointments in a row, but then you will see the cardiologist every third to fourth visit, or more often as required.

 

We have two locations at Health Sciences North at the Ramsey Lake Health Centre (41 Ramsey Lake Road):

Centre Tower – Third floor, Room 3290-3295

If you have been told to come to this location, you will first need to register at Main Registration (main floor) and then have your lab work done before going to your appointment at the Heart Failure Disease Management Program in the Centre Tower. This appointment should take approximately 30 minutes.  Please arrive approximately 30 minutes prior to your booked appointment. Please bring your medications with you.

South Tower – First floor, Medical Day/Infusion Clinic

If you have been told to come to this location, then you will proceed to ACU 1 Registration on the 1st floor, South Tower (behind the south tower elevators). You will register for your appointment. The nurse practitioner will come and get you in the Registration area and bring you to the Medical Day/Infusion Clinic. This appointment should take approximately 60 to 90 minutes. Please arrive approximately 10 minutes before your appointment. Please bring your medications with you.

To speak to the booking clerk about an upcoming test or appointment, please call 705-523-7100, ext. 1221. You may leave a message. Please be aware that there is not always a booking clerk available. The booking clerk may not be able to return your call until the next day.

If you need to speak to someone, please call 705-523-7100, ext. 3662 to reach a Registered Nurse (RN). If there is no answer, you may leave a message and an RN will return your call either that same day or the next business day.

If you require urgent assistance and cannot wait for the booking clerk or RN to get back to you, then please contact your primary care provider, attend a walk-in clinic or seek care at your nearest Emergency Department.
