NEO Navigation Central Intake and Referral Program

About NEO Navigation

NEO Navigation Central Intake and Referral Program is a one-stop-shop (Central Intake) for pediatric healthcare services in Northeastern Ontario. Our multidisciplinary team includes service navigators, nurses, medical secretary, indigenous navigator, and a social workers.

We follow a no-wrong-door approach; helping families and providers navigate the pediatric healthcare system. Each referred client will be individually assessed and connected to services available locally or outside of the community.

Please contact us for information about all NEO Kids and Family programs and services.

Did you know

Many of our services require a valid OHIP card prior to access. You can apply for an OHIP card online or at a Service Ontario location. 

If you are looking for a family doctor or nurse practitioner, you can use Health Care Connect to apply online. 

A number of our specialty clinics and CTC services are eligible for self-referral. You can look for the self-referral icon.

You can fill out the Ocean online self-referral form or contact us for more information.

SmartStart Hub

If you have concerns about your child’s development, if you are not sure what your child’s developmental support needs are, if you are not sure what services your child may need or how to access them - the SmartStart Hub can help you find the help your child may require.

 

All provider referrals can be completed on the NEO Kids & Family Central Intake found on Ocean Healthmap.
