NEO Navigation - Central Intake

NEO Navigation is a one-stop-shop to access pediatric medical healthcare services in northeastern Ontario.

Central intake services are supported by a team of service navigators, a nurse navigator, an indigenous navigator, and a social worker.

NEO Navigation follows a no-wrong-door approach; helping families and providers navigate the pediatric healthcare system. Each referred client will be individually assessed and connected to services that are best for them. NEO Navigation will help support connections to services and supports locally or outside of the community.

Anyone can submit a referral for medical pediatric healthcare services using one of our electronic referral forms.

Anyone requiring information about NEO Kids programs and services, wanting to make a referral to our services or wishing to inquire if a child/youth is eligible for services, may contact NEO Navigation.

As of September 30, 2023, faxed referrals will no longer be available. Any referrals received via fax will be returned to providers. Providers can submit referrals to NEO Navigation using the Ocean e-referral service.

 

Self-Referrals

If you are a patient, parent, guardian or caregiver, and you would like to refer your child to one of NEO Kids & Family programs and services, please fill out the online self-referral form or call the NEO Navigation central intake team at 705-523-7120, and press 1.

General Referrals

Healthcare providers and community agencies can submit an electronic referral for pediatric medical healthcare services through Ocean eReferral. Refer to the NEO Kids & Family Central Intake found on Ocean Healthmap. All NEO Kids & Family outpatient service referrals are embedded in this single eReferral form.

NEO Navigation proudly delivers SmartStart Hub services which connect families to child development services.

If you have concerns about your child’s development, if you are not sure what your child’s developmental support needs are, if you are not sure what services you child may need or how to access them - the SmartStart Hub can help you find the help your child may require.

You do not need a diagnosis to access services. Kids are eligible from birth to age 19, or 21 if they are in school and live in Ontario.
