Children with medical complexity have unique health needs that are not easily met by existing health-care models. Children with medical complexity require care by multiple providers in multiple places over time (ie: specialist care in tertiary pediatric hospitals, children’s treatment centres, primary care and home care).
Complex care programs are an established way of providing care to children with medical complexities. Improved care coordination models help children with medical complexity and their families get the care that they need and improve their care experience.
