HSN’s Bundled and Surgical Virtual Care team has launched a new digital care plan for Kidney Stones through SeamlessMD, marking their 15th pathway!
SeamlessMD is an all-in-one platform for patient engagement, remote monitoring, and virtual care. Patients who enroll in the program can access personalized education, progress-tracking, and post-op symptom monitoring through their smartphone, tablet or computer.
Since the implementation of the platform in 2021, more than 5,350 patients have completed the program and over 160,455 library pages, or 8,022 hours of education, have been provided to patients through the application. Additionally, our teams have reduced length of stay, hospital readmissions, and Emergency Department visits by remotely guiding and monitoring patients throughout their surgical care journey.
