Patients, designated care partners and visitors can screen for COVID-19 before they arrive at the hospital

Patients, designated care partners and visitors are reminded that they can screen for COVID-19 before they arrive at the hospital, to reduce their wait time at hospital entrances.

Using a quick, easy online form, anyone attending the hospital can pre-screen and receive a confirmation email with a QR code. Present your QR code using a paper copy printed at home, or by displaying the code on your mobile device either through your email, or through a photo taken of your QR code.

Self-screening QR codes are only valid for 4 hours after the online form is completed, so please complete the form the same day as your visit to the hospital. Each person entering the hospital should complete their own form and have their own unique QR code for screening. A new form must be completed for each visit.

Patients who have difficulty with the form can come into the hospital and one of our screeners will be happy to assist them in person.
