Welcome to Health Sciences North/Horizon Santé-Nord, (HSN) Cardiac Pulmonary Rehabilitation Program Website.
The website is designed to give you and your family a comprehensive overview of the services and educational opportunities available to you and your family within our program. It is also intended to provided clients in our catchement area, who find it difficult to access our on site services, with valuable educational material and information. The program is based on a scientifically proven and medically supervised course of exercise, education and lifestyle modification aimed at providing patients with the information, motivation and support they need to adopt and maintain heart-healthy habits
Cardiac Pulmonary Rehabilitation teaches patients and families about their disease and how to cope with its physical and psychological symptoms. Programs are available to help patients stop smoking, lower high blood pressure, cholesterol, and reduce emotional stress. Programs are also available to help spouses and family members better accept changes that an acute cardiac event brings. The program is at no cost to you provided there is a referral from your physician or nurse practitioner. Each program referral can be found here (Pulmonary Rehab referral, Cardiac Rehab referral, Adult Asthma Clinic referral).
Cardiac Pulmonary Rehabilitation monitors and improves the recovery, increases fitness, and reduces the risk of further cardiac problems. These goals are accomplished through the multidisciplinary efforts of Nurses, Physicians, Nurse Practitioners, Physiotherapists, Kinesiologists, Psychological Associates, Dieticians, Certified Respiratory Educators, and Certified Clinical Exercise Physiologists (ACSM).
Feel free to look around our new website and we hope that it will provide you with valuable information.
