How do you help patients make the transition from the hospital back to the community, and provide the right care, at the right time, in the right place? What are the gaps between hospital and community-based care, and how can they be filled?
These are some of the questions discussed at the Care Transitions forum presented by HSN, Bayshore Home Health and the North East Community Care Access Centre on Thursday, October 30, 2014.
The Care Transitions forum featured a panel discussion format and was moderated by Steve Paikin, award-winning journalist and author, and best-known as the host of TVO’s flagship current affairs program “The Agenda.”
Other panellists included:
- David Lamb, Manager of Policy for the Transformation Secretariat of the Ontario Ministry of Health and Long-Term Care in charge of the province’s Health Links program
- David McNeil, HSN’s Vice President of Clinical Programs
- Frankie Vitone, Senior Director of Care Coordination for the North East Community Care Access Centre
- Carole Talevi, a patient advocate in Greater Sudbury.
