Complex Care for Kids Ontario (CCKO) is a multi-year provincial strategy to advance province-wide access to integrated medical care and coordination for children with the most complex chronic healthcare needs and their families, starting with Children with Medical Complexity (CMC) who are Medically Fragile Technology Dependent (MFTD).

The CCKO program is contextualized to meet regional circumstances/ecology while underpinned by consistent standards/function and brand across Ontario.

The NEO Kids CCKO satellite clinic serves eligible clients in the Sudbury-Mantioulin District.

To be eligible for Complex Care for Kids Ontario, the child must:

  • Be under 18 years of age.
  • Meets one criterion (indicated by a check box) from four of the five categories below.
  • Medically complex child/youth not currently being followed by a multi-disciplinary team (e.g., diabetes team, cystic fibrosis, or neuromuscular clinics). Rather, child/youth should continue to be followed in their current team rather than (individual exceptions aside) referred to the Complex Care for Kids Ontario.
